The Youth Athletics Incentive Program makes available one $50.00 Dick's Sporting Goods gift card per eligible child who participates in a City of Sunrise youth recreation athletics program. 

The Program, launched in October 2022,  is funded from a portion of the funds received from the American Rescue Plan Act (ARPA) in an effort to expand and enhance participation in the City of Sunrise youth athletics program, to support public health and economic response, and to assist Sunrise residents who have experienced a hardship due to the COVID-19 pandemic. 

Only the parent/guardian of a participant in the following programs is eligible to apply:

  • City of Sunrise Youth Recreation Baseball
  • City of Sunrise Youth Recreation Softball
  • City of Sunrise Youth Recreation Soccer
  • City of Sunrise Youth Recreation Basketball
  • City of Sunrise Youth Recreation Flag Football
  • City of Sunrise Youth Recreation Cheerleading

 

Application Procedures

  1. First, applicants must register their child(ren) by the City’s posted registration deadline for each eligible youth athletics season. Late registrants are not eligible for the Youth Athletics Incentive Program.

  2. Second, City staff will contact all eligible Applicants through email and announce a two-week eligibility period to submit the Eligibility Self-Certification Form. This form can not be completed or submitted prior to the opening of the eligibility period.

  3. Eligibility Self-Certification Forms with residency Proof A, Proof B, or Proof C (chart below) must be submitted in person at the Leisure Services Administrative Office. Once the form is received, City staff will verify eligibility and issue a $50.00 Dick's Sporting Goods gift card per confirmed eligible participating child to Applicant. The gift card is provided to Applicant’s participant(s) to enable the purchase of sporting goods to support participation in the City’s athletics programs.

  4. Applicants must then sign a document to acknowledge receipt of the gift card(s). Please note the following terms of this program:

    • The gift card value may not be redeemed for more than face value, transferred for value, redeemed for cash or returned for a cash refund (except as required by law). The City of Sunrise, Florida is not responsible for gift cards that are lost, stolen, destroyed, or used without permission. The City of Sunrise, Florida is not responsible for, and assumes no liability to you for, any unlawful conduct or fraud by any third party associated with any gift card.

    • The City of Sunrise reserves the right to make changes to the Program or to close the Program at any time, in the City’s sole discretion, as it deems appropriate.

    • The Youth Athletics Incentive Program will be offered to eligible youth athletics program participants on a first come, first served basis. Since this Program is funded by the American Rescue Plan Act funding, once funding is no longer available for this Program, it will be closed.

Eligibility Requirements of the Youth Athletics Incentive Program

  • Applicant must be a Sunrise resident and provide the proof-of-residency documents listed below.

Proof of Residency Requirements

  • Applicant must be the Parent/Legal Guardian of the registered participant(s).
  • Once City staff announce the two-week eligibility period for Applicant must complete an Eligibility Self-Certification Form. Printed copies are available at the Leisure Services Administrative Offices (10610 West Oakland Park Boulevard). Please note: the Eligibility Self-Certification Form can be used for one child or multiple children.
  • Completed Eligibility Self-Certification Forms must be submitted in person at the Leisure Services Administrative Offices. Applicants must bring their Florida Driver License / Florida Photo ID and the documents from residency Proof A, Proof B or Proof C (chart above).
  • Applicants may register more than one participant. Each registered participant is eligible to receive one $50.00 Dick's Sporting Goods gift card per registered athletics program (no maximum award per household).
  • City of Sunrise employees and their children are not eligible for this Program.
